We are seeking a select group of Preferred SNF Partners to join our high-performance network who want to transform the quality of life for their permanent residents while stabilizing facility operations.

High-Acuity Support for Your Medical Director or SNFist

We respect the oversight of your Medical Director or SNFist(s). Synnova provides  a collaborative liaisons for Synnova’s long-term residents:

  • High-Acuity Consulting: We provide extra oversight for complex Synnova members (e.g., oncology, multi-organ failure), acting as a “dedicated assistant” to your Medical Director for these high-risk patients.
  • Shadow Documentation: We manage the heavy lifting of ACO-specific quality reporting and HCC Risk Adjustment, ensuring your residents’ “sickness” is accurately captured to protect the facility’s care-level reimbursement.

24/7 Nursing "Safety Net" for Your Floor Staff

Working alongside your Medical Director and SNFist(s), Synnova delivers a 24/7 clinical safety net for your floor staff and residents:

  • Real-Time Triage: If a Synnova member shows signs of decline at 2:00 AM, your nurses can call the Synnova Rapid Response Line for immediate consultation with a specialized geriatric nurse or NP.

RISE Team: Your Social Work Extension

For residents who do eventually transition to lower levels of care (like Assisted Living), our RISE Team manages the entire hand-off.

  • We provide monitoring, care coordination, and home-safety checks that ensure a successful move, protecting your facility’s 30-Day Readmission Metrics.

MAXIMIZING SNF REVENUE AND BED STABILITY THROUGH THE SYNNOVA LEAD ACO PARTNERSHIP

For our Preferred Provider Network, Synnova ACO introduces The CARA Rewards (Clinical Achievement & Resident Alignment). This program moves beyond traditional per-diem billing, allowing your facility and medical leadership to share in the savings generated by superior clinical management of your long-term residents.

The "Direct-to-Skilled" Internal Waiver

If a long-term resident has a clinical setback (e.g., a fall or a pneumonia flare), you normally have to send them to a hospital for 3 days just to “qualify” for Medicare Part A skilled coverage.

  • The Synnova Advantage: Under the LEAD Model 3-Day Waiver, we can “flip” a long-term resident from custodial to skilled care right in their own bed without a hospital stay.
  • Revenue Protection: This keeps the resident in your facility, earns you the higher skilled rate for the rehab period, and spares the resident the trauma of a hospital transfer.

The "Care-in-Place" Performance Kicker Through our Annual Shared Savings Program

When a long-term resident experiences an acute change in condition, Synnova rewards the facility for providing high-level clinical intervention that avoids a traumatic hospital transfer. As a Preferred Provider, your facility and Medical Director/SNFist are eligible for a performance multiplier within our Annual Shared Savings pool for every hospitalization avoided.
By treating in place, you protect the resident’s continuity of care while sharing in the total cost-of-care savings generated throughout the year.
